Job SummaryAs a Blended Case Manager, you will provide outreach and support to individuals with serious and persistent mental illness, working with them to access and coordinate necessary services. Your primary goal will be to maintain the individual in the least restrictive setting, promoting independence and mental health stability.
Responsibilities- Prepare and maintain accurate and complete records
- Participate as a team member to improve the effectiveness of the work group and achieve group goals
- Deliver interventions and supports in a way that incorporates consistency, flexibility, and responsiveness to changing client needs
- Involve clients in identifying and coordinating treatment issues
- Empower clients through therapeutic rapport, advocacy, and skill building
- Exhibit proficiency with benefit and entitlement options to maximize treatment planning opportunities
- Coordinate emergency psychiatric services in a timely manner, documenting interventions and response
- Bachelor's Degree in a Human Services field, such as social work, psychology, or closely related field
- Commitment to community-based mental health services
- Experience with case management, strongly encouraged but not required
- Computer literacy and knowledge of non-violent crisis intervention principles and practices
- Valid driver's license and record in good standing
