About the Role

We are seeking a compassionate and skilled CNA/Hospice Care Specialist to join our team at Grane Home Health & Hospice. As a key member of our organization, you will play a vital role in providing exceptional patient care and support to individuals in need.

Responsibilities

  • Provide personal care services to patients, including bathing, dressing, and grooming, as assigned.
  • Assist patients with daily living activities, such as meal preparation and ambulation, as instructed by the hospice nurse or therapist.
  • Monitor and report on patient condition and any significant changes to the Case Manager.
  • Participate in quality assessment and performance improvement teams and activities.
  • Adhere to the organization's documentation and care procedures, as well as standards of personal and professional conduct.

Qualifications

  • Meet the training requirements of the State, including a minimum of 75 hours of classroom and supervised practical training.
  • Have a minimum of six months experience in providing personal care, including experience working with the terminally ill.
  • Be at least 18 years of age and possess a high school diploma or equivalent.
  • Have the ability to read and follow written instructions and document care given.
  • Understand the hospice philosophy and be comfortable providing specialized care to the terminally ill.
