About the Role

We are seeking a compassionate and skilled Hospice Nurse Specialist to join our team at Hospice of the Western Reserve. As a key member of our interdisciplinary team, you will provide high-quality patient care and support to individuals and their families in their homes and communities.

Key Responsibilities
  • Assess and address the physical, psychosocial, and spiritual needs of patients and their families
  • Develop and implement individualized care plans in collaboration with the patient's physician and other healthcare professionals
  • Provide skilled nursing services, including pain and symptom management, and emotional support to patients and their families
  • Coordinate with other healthcare providers to ensure seamless care transitions and effective communication
  • Participate in care conferences and bereavement follow-up to support patients and their families throughout the care journey
Requirements
  • Licensed in the State of Ohio
  • Experience in hospice care, home health, oncology, and/or pain and symptom management
  • Current CPR certification
  • Valid driver's license and good driving record
  • Hospice and Palliative Care Certification desired
