We are seeking a compassionate and skilled Pediatric Home Care Nurse to join our team at Hospice of the Western Reserve. As a Pediatric Home Care Nurse, you will provide high-quality, patient-centered care to pediatric patients and their families in the comfort of their own homes.
Key Responsibilities:- Assess the physical, psychosocial, and spiritual needs of pediatric patients and their families.
- Provide intermittent skilled nursing services to pediatric patients under the direction of their physicians.
- Monitor and evaluate pain and symptoms, and provide information and assistance to patients and families to manage those problems.
- Serve as a liaison to pediatric patients' physicians, providing information and support as needed.
- Provide emotional support and education to pediatric patients and their families.
- Refer patients and families to other services as needed, and follow up to ensure continuity of care.
- Maintain accurate and complete records of patient care, including plans and reports.
- Participate in care conferences, bereavement meetings, and other meetings as assigned.
- Licensed RN in the State of Ohio.
- BS or equivalent experience.
- 3-5 years of experience in pediatrics or hospice care, with a desire to transfer into pediatrics.
- Experience in pain and symptom management, oncology, public health, and mental health preferred.
- Valid driver's license, car insurance, and a willingness to work in a variety of settings.
