Kaiser Permanente is seeking a skilled Home Health Physical Therapist to provide high-quality patient care in Sacramento, California.
About the RoleThis position involves making home visits to patients, conducting assessments, and developing personalized treatment plans to help them achieve their goals. As a Home Health Physical Therapist at Kaiser Permanente, you will work collaboratively with patients, families, and healthcare teams to ensure seamless care delivery.
Key Responsibilities- Evaluate patients to determine the nature and extent of dysfunction and develop effective treatment plans.
- Provide one-on-one physical therapy sessions to patients in their homes, teaching them techniques to improve mobility and manage pain.
- Collaborate with other healthcare professionals to coordinate care and ensure comprehensive support for patients.
- Maintain accurate records of patient progress and communicate effectively with patients, families, and healthcare teams.
- Stay up-to-date with industry developments and best practices in physical therapy.
- Graduate of an accredited physical therapy program with a bachelor's, master's, or doctoral degree.
- Minimum two (2) years of experience in physical therapy, preferably in home health settings.
- Clinical skills to detect early warning signs of complications, including medical assessments of skin integrity, mentation, and ability to differentiate emergent versus non-emergent patient care status.
- Able to remove staples/sutures in the home and demonstrate a commitment to service orientation.
- Effective English written and oral communication skills, as well as good interpersonal skills.
- Experience working with automated clinical information systems and being computer literate.
- Knowledge of Medicare conditions of participation and OASIS.
- Assessment skills for adaptive equipment for Activities of Daily Living.
We offer a competitive salary range of $120,000 - $160,000 per year, depending on experience, plus benefits including medical, dental, and vision coverage, 401(k) plan, and opportunities for professional growth and development.
