Home Health Physical Therapist Assistant

We are seeking a skilled Home Health Physical Therapist Assistant to join our team at All Medical Personnel. As a Home Health Physical Therapist Assistant, you will work collaboratively with patients and their families to develop personalized plans of care that focus on achieving optimal physical therapy outcomes.
You will provide high-quality physical therapy services in the patient's home, promoting independence and enhancing their quality of life. This is an excellent opportunity for a dedicated and compassionate professional to make a meaningful difference in the lives of our patients.
Key Responsibilities- Collaborate with interdisciplinary teams to develop holistic plans of care focused on achieving patient-centered goals.
- Instruct, coach, and guide patients and their families to support them in achieving optimal outcomes while remaining in their communities.
- Promote optimal health, well-being, and quality of life of patients and their families by providing skilled, intermittent physical therapy in the patient's home.
- Practice independently in the patient's home using evidence-based practice for assessment, clinical decision-making, and coordination of care across the continuum.
- A minimum of 1-2 years of experience in an outpatient rehabilitation setting.
- At least two (2) years of physical therapy experience is highly preferred.
- One (1) year of experience in home health or other health settings with a geriatric emphasis.
- Current license in Physical Therapy or ability to obtain prior to hire.
- American Heart Association Healthcare Provider BLS certification preferred upon hire and required within 90 days of hire.
- Current Driver's license.
- Excellent customer service skills.
- Ability to work without direct supervision.
- Provides care appropriate to the age of patients served.
