Overview

We are looking for a Registered Nurse - On Call to join our team. This salaried position reports directly to the Administrator, Executive Director or Patient Care Manager Senior and will maintain primary control and professional management of each patient.

Schedule: 7 on/7 off

The ideal candidate will act as primary liaison between physician, patient/family and hospice team. Key responsibilities include providing direct patient care services to hospice patients and supportive care to the patient's family after normal business hours on an on-call basis.

Job Responsibilities:

  • Evaluate patient/family needs and assist in the preparation and execution of the plan of care for each patient
  • Manage an interdisciplinary group consisting of Licensed Practical/Vocational Nurses, Certified Nursing Assistants and support staff to enhance the quality of hospice care
  • Maintain all assigned patient clinical records
  • Participate in company quality assessment and performance improvement programs

About You

We seek a compassionate, kind and empathetic individual who is a current RN license holder (in the state of requested employment or ability to obtain). Preferably, you have 1+ years of nursing experience in hospice, home health, or oncology.
