Job Summary

The MDS Coordinator will be responsible for ensuring the timely and accurate completion of the RAI process and care management process from admission to discharge in accordance with company policy and procedures, as well as Federal, State, and Certification guidelines.

Key Responsibilities
  • Collaborate with the Interdisciplinary Team to assess resident needs and provide interdisciplinary schedules for MDS assessments and care plan reviews.
  • Ensure the Interdisciplinary team makes decisions for completing or not completing additional MDS assessments based on clinical criteria.
  • Coordinate and manage the daily stand-up meeting to review resident care and set assessment reference dates.
  • Comply with federal and state regulations regarding the completion and coordination of the RAI process.
  • Monitor MDS and care plan documentation for all residents and ensure documentation is present in the medical record to support MDS coding.
  • Maintain current MDS status of assigned residents according to state and federal guidelines.
  • Enter resident information into computerized MDS programs and complete accurate coding of the MDS.
  • Attend interdisciplinary team meetings, quality assurance meetings, and other meetings to gather information and maintain records.
  • Assist the DON or designee in identifying significant changes, physician orders, and verbal reports to ensure the MDS and care plan are reflective of those changes.
  • Prepare scheduling, notice of resident care planning conferences, and assist the DON in communicating outcomes and problems to responsible staff, residents, and/or responsible parties.
  • Continually update knowledge base related to data entry and computer technology.
  • Submit required documentation to the state database and other entities per company policy.
  • Correct and ensure completion of final MDS and submit resident assessment data to the appropriate State and Federal government agencies.
  • Assign, assist, and instruct staff in the RAI Process, PPS Medicare, Medicaid, and clinical computer system in relation to these processes.
  • Maintain confidentiality of necessary information.
Qualifications
  • Graduate of an approved Registered Nurse program and licensed in the state of practice, required.
  • Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.
  • Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement, as required.
  • Thorough understanding of the Quality Indicator process and knowledge of the OBRA regulations and Minimum Data Set.
  • Knowledge of the care planning process.
  • Experience with MDS 3.0, preferred.
