- Implement strategies to minimize medication-related complications as patients transition through various healthcare environments.
- Participate in initiatives aimed at improving health outcomes for chronic conditions such as COPD, Asthma, CHF, and Diabetes.
- Deliver comprehensive medication therapy management, including patient counseling and medication history assessments.
- Monitor and report medication errors and adverse drug reactions, developing preventive measures.
- Engage in quality improvement projects and research to enhance pharmaceutical care practices.
- Conduct medication use evaluations to ensure cost-effectiveness and appropriateness of therapy.
- Facilitate medication reconciliation across the continuum of care to prevent readmissions and lapses in treatment.
- Develop clinical guidelines and protocols to standardize care practices within the institution.
- Provide specialized education and training to healthcare professionals in advanced pharmacotherapy.
- Oversee both clinical and distributive functions of the pharmacy department as necessary.
For Level I: Valid NY State license to practice pharmacy.
For Level II: Two years of post-licensure experience or completion of a postgraduate specialty residency or fellowship program.
For Level III: One year of post-licensure supervisory experience or specialty clinical experience in addition to the above qualifications.
