The Opportunity

We are seeking a Health Coach to join our team at one of our offices focused on our Senior Health member population (age 65+). As a Health Coach, you will be an integral part of a primary care team that supports patients in meeting their preventive, chronic, and acute care needs.

Your primary responsibilities will include establishing trusting relationships with patients and their families, advocating for their needs, and collaborating with an integrated primary care team to facilitate better healthcare outcomes.

You will work closely with patients to capture relevant information about their health and healthcare experience, assist with coordination of post-visit care, and participate in daily operations of the primary care practice.

As a Health Coach, you will also provide patient education on chronic disease management, coach patients using motivational interviewing techniques, and provide anticipatory preventive guidance to families with children.

What You'll Need
  • At least 1 year of experience in a high-touch customer service or patient-facing role in a healthcare setting
  • Strong written and verbal communication skills
  • Experience working on collaborative, diverse, and feedback-driven multi-disciplinary teams
  • A proven track record of persisting through change, demonstrating a forward-thinking perspective when under pressure, and consistently stepping up to take action on challenges
  • Proficiency in computer technology, including typing, navigating the internet, and using multiple software systems simultaneously

Competitive Salary

The base pay for this position is $24.25 per hour based on a full-time schedule.

We also offer a 401K match, credit towards emergency childcare, extra contributions toward maternity and paternity leave, paid Life Insurance, and Disability insurance.

This is a full-time role (40 hours/week, Monday-Friday, 8am-5pm) based in Hyannis, MA at one of our offices focused on our Senior Health member population (age 65+).
