Job Summary:

The Arizona Pioneers' Home is a premier continuing care retirement home, operated and funded by the State of Arizona. Our mission is to provide exceptional care and services to residents living independently, as well as those requiring personal, assisted living, intermediate, and skilled care.

Key Responsibilities:
  • Administer medication treatment in a safe and timely manner, adhering to all applicable rules, regulations, laws, policies, and procedures.
  • Document and communicate residents' care and condition to the interdisciplinary team and other healthcare settings, ensuring continuity of care and adherence to HIPAA regulations.
  • Perform assessments, prepare incident reports, and participate in QAPI projects to improve care, as requested.
  • Supervise nursing assistants to ensure quality care and adherence to AZPH policies and procedures.
  • Resolve interpersonal conflicts, work procedures issues, and answer questions regarding care, while assisting in training and reinforcement of care needed during a shift.
  • Function as an advocate for residents, ensuring quality care and respect for their rights, wishes, and directives.
  • Present a professional demeanor and interact with residents, coworkers, residents' families, visitors, volunteers, and vendors, maintaining confidentiality of protected health information.
  • Perform assessments and document findings to physicians, as indicated.
  • Initiate, review, and update resident care plans.
  • Document care provided in medical records.
Requirements:
  • Valid fingerprint clearance card or application to be submitted within twenty days of hire to the Department of Public Safety.
  • License to practice as a Licensed Practical Nurse in the State of Arizona.
  • Current CPR Card and First Aid.

Retirement:

Positions in this classification participate in the Arizona State Retirement System (ASRS). Enrollment eligibility will become effective after 27 weeks of employment.
