About the Role:
We are seeking a dedicated and compassionate Primary Care Advanced Practice Clinician to join our team at Laurinburg Area Post-Acute Care Practice in Laurinburg, NC. As a key member of our interdisciplinary team, you will play a vital role in delivering high-quality primary care services to our patients in post-acute care communities.
Responsibilities:
- Provide comprehensive primary care services to a manageable patient panel of 15-20 patients per day
- Collaborate with interdisciplinary teams to optimize patient outcomes
- Ensure smooth transitions of care with hospice, palliative care, and home care agencies
- Maintain accurate and timely documentation in our EMR system (Gehrimed)
