About the Role

The Case Manager - Housing Stability Specialist will play a critical role in supporting individuals and families at risk of homelessness or recently housed. This position requires a self-starter with excellent communication and problem-solving skills.

Key Responsibilities
  • Provide ongoing, community-based services to assigned clients, including assessment, goal setting, and service planning.
  • Identify and prioritize client needs, developing strategies to address immediate and long-term goals.
  • Support clients in developing money management skills, finding gainful employment, and accessing education and training opportunities.
  • Coordinate with community partners to provide wrap-around services, including mental health and substance abuse support.
  • Ensure clients achieve housing stability by complying with lease agreements and maintaining individualized housing sustainability plans.
  • Moderate household, roommate, and landlord-tenant disputes that threaten housing stability.
  • Develop and maintain a resource directory of state and local service providers.
  • Maintain thorough case documentation and comply with agency and standard best practices.
Requirements
  • Bachelor's Degree in Social Work, Psychology, Human Development, or related field.
  • Three years of experience in service delivery with low and moderate-income families.
  • Ability to prioritize competing priorities and make sound judgments.
  • Strong written communication skills.
  • Ability to advocate, assist in problem-solving, and achieve results for families.
  • Excellent time management and organizational skills.
  • Valid Driver's License.
  • Willingness to work evenings and weekends as needed.
Preferred Qualifications
  • Master's Degree in Social Work or related field.
  • Experience providing services to clients experiencing homelessness, mental health, or substance abuse issues.
  • Ability to speak a language other than English at a proficient level.
  • Comfortable with and quickly adapts to change.
  • Strong public speaking and communication skills.
