Director of Discharge Planning and Social Services

Charlottesville Health & Rehabilitation Center is seeking an experienced professional to lead our discharge planning and social services team. As a key member of our staff, you will play a vital role in ensuring the smooth transition of our residents to their next level of care.

Responsibilities:
  • Coordinate discharge planning meetings and arrange external services for residents, including chaplaincy, advocacy, and home care.
  • Assess patient support systems and home situations to inform discharge planning decisions.
  • Facilitate effective bed management and maximize availability of transitional and extended care services.
  • Meet with residents to address social issues, such as conflict resolution and care coordination.
  • Conduct customer satisfaction surveys to ensure high-quality care.
Requirements:
  • Bachelor's degree in Social Work, Psychology, Sociology, Education, Counseling, or a related field.
  • At least 1 year of experience in social work or a related field in a healthcare setting.
