About the Role

In this role, you will have the opportunity to work with a team to provide care to patients at home, in nursing homes, assisted living facilities, and senior housing. You will conduct comprehensive assessments, manage patients through acute medical situations, and ensure accurate and complete documentation.

Key Responsibilities
  • Conduct comprehensive assessments and develop patient care plans
  • Manage patients through acute medical situations and exacerbations of chronic conditions

Requirements
