Licensed Practical Nurse

Fortcom Health Services is seeking a Part-Time Licensed Practical Nurse (LPN) to join our team. As a home health nurse, you will be responsible for providing high-quality patient care in the comfort of their own homes.
Key Responsibilities:- Conduct thorough clinical assessments and evaluations to develop personalized care plans.
- Train, evaluate, and supervise AFC caregivers to ensure exceptional patient care.
- Monitor patient health status and report any changes to the client's physician.
- Provide individualized on-site nursing care to patients, utilizing a comprehensive and client-centered Plan of Care.
- LPN license with a minimum of two years of previous nursing experience.
- Must be fully licensed RN by the Massachusetts Board of Registration.
- Bilingual skills in Spanish or Haitian Creole are a plus.
