Job Opportunity:

Key Responsibilities:
  • Collaborate with physicians and healthcare professionals to deliver exceptional patient care and services.
  • Enroll patients in specialty programs, ensuring optimal clinical outcomes.
  • Provide education and training to pharmacy teams on specialty services and offerings.

Patient Care and Operations:
  • Conduct and support pharmacy team conversion conversations with patients, ensuring thorough documentation of achieved outcomes.
  • Provide initial clinical medication counseling and regular patient reassessments to ensure appropriateness of specialty therapy.
  • Perform medication therapy and disease state management, including coordinating medication therapy adjustments.
  • Refer patients to financial counseling for copay assistance through manufacturer or charity programs.
  • Apply principles of continuous quality improvement consistent with job expectations.
  • Communicate with patients and patient families about the benefits of filling their prescriptions with Baptist Health.

Requirements:
  • Kentucky and/or Indiana Board of Pharmacy license in good standing. Licensure in surrounding states may be necessary or preferred.
  • Bachelor's Degree in Pharmacy or Doctorate of Pharmacy required.
  • 1-3 years related work experience or completion of ASHP accredited pharmacy residency program preferred.
  • Experience in ambulatory clinical pharmacy practice preferred.
  • Disease state specialty experience preferred.
  • Possesses knowledge in drug information, third-party claim adjudication, and regulatory requirements of pharmacy practices.
