Job Summary

As a Patient Care Associate at Northwell Health, you will play a vital role in providing direct care to patients, ensuring their comfort, safety, and well-being. This is an exciting opportunity to join our team and make a meaningful difference in the lives of our patients and their families.

Key Responsibilities
  • Assist with patient admissions and discharges, ensuring a smooth transition of care.
  • Establish and maintain effective relationships with patients, families, and healthcare teams.
  • Communicate vital information to the healthcare team, ensuring seamless care coordination.
  • Convey important information to patients and visitors, promoting transparency and understanding.
  • Maintain open communication with colleagues to ensure optimal team functioning.
  • Document patient vital signs, including temperature, blood pressure, respirations, pulse, weight, and height, using appropriate equipment.
  • Report significant changes in patient condition to the Registered Nurse and healthcare team.
  • Communicate effectively, both verbally and in writing, to ensure clear understanding and action.
  • Respond promptly to patient requests and call lights, providing timely support and care.
  • Maintain a safe, comfortable, and therapeutic environment for patients and families, adhering to hospital standards.
  • Function effectively in emergency situations, working collaboratively with the healthcare team.
  • Assist with restrained patients, following established policies and procedures.
  • Perform decubitus care, as directed, to promote patient comfort and prevent pressure ulcers.
  • Assist with activities of daily living, tailored to the patient's condition and needs.
  • Use proper body mechanics, seeking assistance and utilizing equipment when necessary, to prevent injury and promote patient safety.
  • Perform delegated patient care procedures, ensuring accurate and timely completion.
  • Obtain and label specimens, performing diagnostic tests as directed, to support patient care and diagnosis.
  • Perform phlebotomy procedures, ensuring patient comfort and safety.
  • Conduct telemetry and electrocardiograms, as directed, to support patient care and diagnosis.
  • Apply clinical knowledge to care for patients, based on diagnoses and unit assignments.
  • Perform related duties as required, ensuring seamless care coordination and patient satisfaction.
Qualifications
  • High School Diploma or equivalent required.
  • 1-3 years of relevant experience, required.
