Job Summary:

EvergreenHealth is seeking a skilled LPN to provide high-quality nursing services to patients in their homes. As a Home Health Nurse LPN, you will work under the direct supervision of a registered nurse to develop and implement personalized care plans for patients. Your primary duties will include providing direct patient care, coordinating patient and family education, and communicating effectively with healthcare teams.

Responsibilities:

Provide direct patient care in the patient's home, following established standards of care and hospital guidelines.
Coordinate patient and family education, incorporating patient and family into the care planning process.
Participate in interdisciplinary team meetings to develop and implement patient care plans.
Communicate effectively with healthcare teams, including physicians, nurses, and other healthcare professionals.

Requirements:

Currently licensed as a LPN in the State of Washington.
Current Washington State Drivers license and proof of insurability.
Current Healthcare Provider BLS certification by date of hire.
Reliable transportation.
