Program Guide PACE Adult Day Center Job Description

About the Role:

We are seeking a dedicated and compassionate Program Guide to join our team at CarePartners Health Services. As a Program Guide, you will play a vital role in supporting the daily activities and care of our participants in the PACE Adult Day Center.

Key Responsibilities:

  • Assist participants with personal care needs, including showers, incontinence, and dressing.
  • Support participants with snack and meal preparation and service.
  • Coordinate environmental and supply needs to ensure a comfortable and engaging experience for participants.
  • Participate in and support Quality Improvement initiatives to continually improve care for participants.
  • Maintain accurate and timely records of participant care and activities.

Requirements:

  • High school diploma or equivalent required.
  • Previous experience in a healthcare or social services setting preferred.
  • Ability to work in a fast-paced environment and prioritize multiple tasks.
  • Excellent communication and interpersonal skills.
  • Ability to maintain confidentiality and handle sensitive information.
