About the Role:
We are seeking a dedicated and compassionate Program Guide to join our team at CarePartners Health Services. As a Program Guide, you will play a vital role in supporting the daily activities and care of our participants in the PACE Adult Day Center.
Key Responsibilities:
- Assist participants with personal care needs, including showers, incontinence, and dressing.
- Support participants with snack and meal preparation and service.
- Coordinate environmental and supply needs to ensure a comfortable and engaging experience for participants.
- Participate in and support Quality Improvement initiatives to continually improve care for participants.
- Maintain accurate and timely records of participant care and activities.
Requirements:
- High school diploma or equivalent required.
- Previous experience in a healthcare or social services setting preferred.
- Ability to work in a fast-paced environment and prioritize multiple tasks.
- Excellent communication and interpersonal skills.
- Ability to maintain confidentiality and handle sensitive information.
