About the Role:

We are seeking an Advanced Healthcare Professional to join our team at Collage Home and Community Rehabilitation Partners in Texas. As a dedicated member of our interdisciplinary team, you will provide patient-centered care to individuals with traumatic brain injuries, spinal cord injuries, CVAs, and other neurological or complex orthopedic diagnoses.

Key Responsibilities:

  • You will assess patient needs and develop personalized treatment plans
  • Counsel patients and families on adjusting to disability and linking to resources
  • Direct medically complex patients in modifying behavior

Requirements:

  • A Master's degree in Social Work and state licensure as an LCSW are required
  • Excellent planning, organization, writing, and time management skills are essential
  • Ability to communicate effectively with families, professionals, and colleagues is crucial
