Job Description
We are seeking a highly skilled Healthcare Professional to join our clinical team as a Licensed Practical Nurse (LPN). The successful candidate will be responsible for delivering efficient and effective nursing care to our patients, evaluating their conditions, collecting data, and reporting changes to the healthcare team. You will also contribute to nursing assessments and care planning, supervise and monitor patient care provided by unlicensed staff, and administer medications as prescribed by physicians.

Key Responsibilities
* Provide high-quality patient care, achieving positive clinical outcomes and patient/family satisfaction
* Collect data, observe patient conditions, and report changes to the healthcare team
* Contribute to nursing assessments and care planning, ensuring comprehensive care delivery
* Supervise and monitor patient care provided by unlicensed staff, promoting a safe and supportive environment
* Administer medications and perform treatments as prescribed by physicians, maintaining patient safety and confidentiality

Requirements and Qualifications
* Graduate of an approved school for practical or vocational nursing
* Currently licensed by the State Board of Nursing or pending licensed practical nurse licensure
* Experience in rehabilitative or geriatric nursing preferred
* Maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility
