POSITION SUMMARY:
- Delivers empathetic counseling, daily living skills training, crisis management, and rehabilitative services to individuals with mental health challenges in residential settings, adhering to applicable regulations.
KEY RESPONSIBILITIES:
- Facilitate counseling sessions, behavioral support, rehabilitative services, and aid in community engagement and crisis resolution.
- Oversee, instruct, and support residents in maintaining a clean, safe, and welcoming living environment, actively participating in household upkeep as necessary.
- Provide coverage across all residential locations as required.
- Guide residents in daily tasks such as grocery shopping, leisure activities, meal preparation, and personal hygiene.
- Assist residents in formulating meaningful goals within their service plans and offer support to help them achieve these objectives.
- Conduct fire safety training and ensure compliance with fire drill regulations.
- Engage in all Medicaid billable activities under the direction of the Supervisor, ensuring all documentation meets regulatory standards.
- Record interactions with residents in accordance with agency policies and procedures.
- Meet regularly with assigned residents for counseling and to review service plan objectives.
- Maintain communication with external service providers and participate in scheduled case meetings.
- Support residents in obtaining and managing financial benefits.
- Monitor medication administration as required.
- Conduct scheduled visits to residents in their homes and/or provide 24-hour supervision in communal living facilities.
- Offer transportation as needed, utilizing personal vehicles when agency vehicles are unavailable.
- Report all emergencies, incidents, and safety concerns to the senior Counselor or Supervisor without delay.
- Participate in team meetings as scheduled.
- Encourage data sharing with relevant individuals to promote collaborative decision-making and keep clients informed of their progress.
- Complete Quality Improvement (QI) Training during onboarding and engage in refresher training as necessary.
- Identify and implement quality enhancement training and activities as part of professional development objectives.
- Foster a culture of inclusion that values the diverse strengths, perspectives, and experiences of all employees, enhancing decision-making and innovation.
- Adhere to all safety protocols to ensure a secure working environment for all staff, demonstrating a commitment to safety by following established guidelines.
- Other duties as assigned.
EDUCATION & EXPERIENCE:
- High School Diploma required.
- Preferred: Minimum of 5 years of experience in a mental health-related field.
