Key Responsibilities:- Collaborate with interdisciplinary teams to review patient activities, medical necessity, and length of stay to ensure cost-effective outcomes.
- Develop and implement individualized care plans to enhance physical, psychosocial, and vocational health.
- Coordinate and evaluate health-related service options throughout the continuum, ensuring timely and appropriate utilization of resources.
- Provide expert clinical guidance and support to patients, families, and caregivers to promote informed decision-making and self-advocacy.
- Stay up-to-date with national standards, best practices, and regulatory requirements to ensure compliance and excellence in case management.
- Associate Degree in Nursing, required; Bachelor's Degree in Nursing, preferred.
- Current Texas licensure with State Board of Nurse Examiners as an RN.
- 1+ years of experience in healthcare, preferably in case management or a related field.
- Case Management certification, preferred.
