About the Role

The Clinical Health Specialist will conduct adult/geriatric assessments, medical history reviews, diagnosis, and treatment planning. They will also provide health education, referrals to physicians and case managers, and follow-up documentation according to Matrix guidelines and protocols.

This role requires strong communication skills, both verbal and written, with patients, clients, and colleagues. The ability to travel and meet patients in their homes is essential.

Key Responsibilities
  • Conduct comprehensive assessments, medical history reviews, diagnosis, and treatment planning
  • Provide health education and referrals to physicians and case managers
  • Document all interactions and assessments according to Matrix guidelines and protocols
  • Collaborate with healthcare teams to ensure seamless patient care
Requirements

To be successful in this role, you will need:

  • A Master's degree in a relevant field or equivalent experience
  • Certification as a Nurse Practitioner or Physician Assistant
  • Current licensure in good standing
  • Board certification in a Matrix-approved specialty
  • Experience working in a healthcare setting, preferably with geriatric populations
  • Strong computer skills and familiarity with electronic medical records software
Compensation and Benefits

We offer competitive compensation, including an estimated salary range of $2,300-$11,500 per month, based on completed visits and location. Additionally, we provide mileage reimbursement, 401(k) matching, and limited voluntary benefits.

Travel Requirements

This role requires regular travel to meet patients in their homes, with some flexibility to accommodate varying schedules. A valid driver's license and adequate automobile insurance are required.
