Job Title: Bilingual Community Support Worker

At Community Health Resources, we are seeking a highly motivated and compassionate Bilingual Community Support Worker to join our team. As a key member of our Community Support for Families program, you will play a vital role in providing case management services to families with reports of abuse/neglect, coordinating child and family teaming activities, and offering an array of services/supports to enhance protective factors.

About the Program

Our Community Support for Families program aims to provide comprehensive support to families in need, ensuring they have access to the resources and services necessary to thrive. As a Bilingual Community Support Worker, you will work closely with families to assess their strengths and needs, develop personalized plans, and connect them with community resources.

Key Responsibilities
  • Assess whole family needs, including basic needs, therapeutic, medical, and other needs
  • Provide support and crisis management related to family needs
  • Assist in linking families with needed community support services, programs, and agencies; advocate for clients' rights and access to services
  • Work as liaison with DCF, hospitals, other community-based services, family members, and significant others
  • Process appropriate requests for flex funding
Qualifications

We are seeking candidates with a Bachelor's Degree in a Human Services related field, or an Associate's degree plus an extra two years' experience in a related field with an approved waiver from funder DCF. A minimum of 2 years experience in mental health related setting is required. Bilingual in English and Spanish is a must.
