Position Overview:
The Wellness Program Coordinator is essential in supporting American House's commitment to enhancing the quality of life for our residents. This role involves assisting in the management, training, and guidance of all Wellness team members. The Coordinator also assumes leadership responsibilities in the absence of the Wellness Director.

Key Responsibilities:

  • Manage the scheduling of staff, including Resident Assistants and Medication Technicians.
  • Engage in ongoing quality improvement initiatives by ensuring accurate documentation and record-keeping.

    • Collaborate in the development and execution of care plans with residents, their families, healthcare providers, and the Wellness Director.

      • Administer medications in accordance with state regulations and established protocols as necessary.

        • Monitor residents' health and report any changes in their physical or emotional well-being to the Wellness Director.

          • Maintain a clean, safe, and organized living environment for residents, ensuring that housekeeping tasks are completed daily.

            • Assist residents with daily living activities, including mobility, personal hygiene, and nutrition, in line with their individual care plans.

              • Facilitate resident engagement in activities, encouraging social interaction and participation in community events.

                • Communicate effectively with healthcare professionals and external service providers.

                  • Adhere to safety protocols, including infection control and emergency response procedures, while promptly addressing requests for assistance.

                    • Report any concerns regarding resident welfare or safety to the appropriate authorities.

                      • Attend Wellness meetings and training sessions to ensure compliance with all relevant regulations and policies.

                        • Support the Wellness Director in overseeing the Wellness team, including recruitment, training, and performance management.

                          • Evaluate potential residents for appropriate care levels using established assessment tools.

                            • Ensure adherence to state regulatory requirements in the absence of the Wellness Director.

                              • Complete and file incident reports as directed, ensuring appropriate follow-up actions are taken.

                                • Maintain compliance with safety regulations and medication management policies as delegated by the Wellness Director.

Preferred Qualifications:

  • Minimum of three years of management experience in a healthcare or service-oriented environment.
  • Experience working with seniors and individuals experiencing memory challenges.
  • Licensed Practical Nurse (LPN) or Registered Nurse (RN) credentials.

Education and Requirements:

  • Certification as a Nursing Aide and Medication Technician in the relevant state.
  • Must be at least 18 years of age.
  • Successful completion of a background check, drug screening, TB test, and physical examination.

Physical and Mental Requirements:

  • Ability to stand, walk, bend, and lift items regularly.
  • Capability to assist residents with daily activities, including bathing and dressing.

    • Effective communication skills to interact with residents, families, and colleagues.

      • Specific vision abilities required for this role include close vision and depth perception.


Work Environment:

  • Interaction with coworkers, clients, and vendors is required.
  • Flexibility to work occasional weekends, evenings, or nights as needed.
  • Potential exposure to communicable diseases and other health risks.
  • Risk of injury when assisting residents with mobility and transfers.
  • Possible exposure to various chemicals and biological hazards.
