Patient Care Liaison for Primary Care Practice

We are seeking a highly skilled and compassionate Patient Care Liaison to join our team at DMC Primary Care. As a key member of our healthcare delivery team, you will be responsible for delivering high-quality administrative tasks in close collaboration with our clinical care teams.
Key Responsibilities:- Monitor and Sort Electronic Medical Records: Ensure timely completion of messages in accordance with DMC Policies & Procedures.
- Communicate Patient Results: Relay results to patients including labs, imaging, and other testing.
- Coordinate Patient Scheduling: Schedule appointments at DMC and with specialists and outside facilities.
- Document Patient Communication: Document all patient communication appropriately using professional etiquette and technique.
- Stay Up-to-Date with EMR System: Maintain proficiency in Allscripts EMR system, clinic flow, and documentation standards.
- High School Diploma or Equivalent: Required.
- Administrative or Clinical Experience: One year of experience in an administrative or clinical position in a medical office is preferred.
- Medical Terminology Knowledge: Knowledge of medical terminology is preferred.
