Job Summary:

Community Health Resources is seeking a highly motivated and compassionate Recovery Support Specialist to join our team. As a Recovery Support Specialist, you will play a vital role in providing outreach, engagement, and support to individuals struggling with substance use or co-occurring disorders.

Key Responsibilities:

  • Provide recovery coaching and support to individuals in need, connecting them to community resources and services.
  • Develop and implement individualized recovery plans, working closely with clients to identify and achieve their goals.
  • Collaborate with hospital-based staff, EMS, law enforcement, and community providers to ensure seamless care coordination.
  • Participate in interventions to reduce healthcare disparities, including screening for Social Determinants of Health.
  • Provide peer support, consultation, and discharge planning to clients at risk of or presenting to an Emergency Department.

Requirements:

  • Recovery Support Specialist Certification from Advocacy Unlimited or equivalent required.
  • Strong understanding of substance use disorders, recovery principles, and community resources.
  • Excellent communication and interpersonal skills, with ability to work effectively with diverse populations.
  • Ability to work independently and as part of a team, with strong problem-solving and critical thinking skills.
