Job Responsibilities
  • Providing high-quality patient care as a Licensed Practical Nurse (LPN) in post-acute facilities
  • Following and adhering to resident care plans to ensure optimal outcomes
  • Monitoring the wellbeing of residents, including physical and mental health, and tracking vital signs
  • Conducting intakes, documenting health concerns and medical history, and administering medications
  • Reviewing resident charts and medical records to ensure accurate and up-to-date information
  • Assisting with the cultivation of a safe and comfortable facility environment
  • Maintaining appropriate stocks of medical supplies and participating in IV set up, immunizations, and phlebotomy procedures
  • Helping with the orientation and training of medical staff
Qualifications and Skills
  • Current Pennsylvania state license to practice as a Licensed Practical Nurse (LPN)
  • Completion of an approved Licensed Practical Nurse (LPN) educational program
  • High school diploma (or GED/equivalent)
  • Minimum of 6 months of experience as an LPN or in a related role
  • Ability to lift a minimum of 50 lbs
