Position Title:
LTSS Service Coordinator - RN Clinician

Job Overview:
This role involves fieldwork, and candidates should be based in the designated service areas.
The LTSS Service Coordinator RN Clinician is tasked with the comprehensive oversight of a member's case within the boundaries of their licensure. This includes providing guidance and support to non-RN clinicians involved in the member's care, in compliance with relevant state regulations and contractual obligations. The coordinator is responsible for developing, monitoring, assessing, and adjusting the member's care plan to ensure it aligns with their needs, aiming to enhance the member's healthcare experience throughout the care continuum.

Key Responsibilities:
- Identify members at risk for complications and facilitate coordinated care with the member and healthcare team.
- Manage individuals with chronic conditions, co-morbidities, and disabilities to ensure effective and efficient use of health benefits.
- Conduct thorough assessments to gather comprehensive member histories for individualized care planning.
- Collaborate with members, caregivers, and healthcare providers to establish both short-term and long-term care goals.
- Ensure that members have access to necessary services as outlined in their care plans.
- Assist in the execution of care plans by facilitating necessary authorizations and referrals within the benefits framework.
- Collaborate with Medical Directors and Inter-Disciplinary Teams to create effective care management strategies.
- Supervise the activities of LPNs, LSWs, LCSWs, LMSWs, and other licensed professionals, ensuring appropriate task delegation and performance evaluation.

Minimum Qualifications:
- Must possess an RN license and have at least three years of experience working with individuals facing chronic illnesses, co-morbidities, or disabilities in roles such as Service Coordinator or Case Manager, or possess equivalent education and experience.
- A current, unrestricted RN license in the applicable state(s) is mandatory. Additional state-specific certifications may be required based on regulations.

Preferred Qualifications:
- A Master's degree in Health or Nursing is preferred. Willingness to travel to various work sites as necessary.
