Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our Home Based Primary Care (HBPC) team at the VA Northeast Ohio Healthcare System. As an LPN, you will play a vital role in providing high-quality patient care to our nation's veterans in the comfort of their own homes.

Key Responsibilities
  • Provide direct patient care and education to veterans with complex medical and psychosocial conditions.
  • Collaborate with the HBPC interdisciplinary team to develop and implement individualized plans of care.
  • Conduct home visits to assess patient needs and provide care and education.
  • Work closely with other healthcare professionals to ensure seamless care coordination.
  • Participate in ongoing education and training to stay up-to-date on best practices and evidence-based care.
Requirements
  • Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or accredited by the National League for Nursing.
  • Valid driver's license.
  • Current licensure as a licensed practical or vocational nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or District of Columbia.
  • At least one year of qualifying experience at the GS-6 level or equivalent.
Preferred Qualifications
  • 2 years of nursing experience in skilled home care, hospice, adult inpatient setting or skilled nursing facility.
  • Excellent interpersonal relationship skills.
  • Ability to work in an interdisciplinary setting.
  • Ability to work independently and make competent clinical decisions using critical thinking skills.
  • Sound documentation skills.
  • Ability to prioritize responsibilities.
  • Ability to work independently in a variety of home environments.
