Job Description
Mercy Health seeks a skilled Patient Transition Manager to join our team. This role is responsible for coordinating patient care to ensure seamless transitions across various care settings. The ideal candidate will have a strong understanding of care coordination principles and be able to work effectively with multidisciplinary teams.

About the Position
This is a full-time position responsible for providing coordination of care for patients to support safe, timely transitions across the continuum. Key responsibilities include developing and implementing individualized care plans, collaborating with multidisciplinary teams, maintaining accurate documentation, fostering relationships with patients and families, and working closely with revenue cycle partners.

Key Responsibilities:
  • Assess patients' biopsychosocial, functional, cultural, spiritual, and financial needs to develop individualized care plans.
  • Collaborate with multidisciplinary teams to ensure effective communication and coordination of patient care.
  • Maintain accurate, concise, and timely documentation in patient records to reflect their physical and functional limitations, psychosocial characteristics, educational needs, financial constraints, and transitional requirements.
  • Build relationships with patients, families, and caregivers to promote assertive, proactive care and remove barriers related to achieving timely testing and treatment.
  • Work closely with revenue cycle partners to ensure accurate classification and coding of patient information.
  • Participate in nursing unit and department clinical outcome projects as well as process improvement initiatives within care management.

Salary and Benefits
We offer a competitive salary of $67,000 - $87,000 per year, depending on experience, and a comprehensive benefits package, including medical, dental, and vision plans, prescription drug coverage, flexible spending accounts, life insurance w/AD&D, employer contributions to retirement savings plan when eligible, paid time off, and much more.
