Job Summary:

We are seeking a skilled Nurse Care Manager to join our team at Mercy Health. In this role, you will be responsible for coordinating patient care across the healthcare continuum, ensuring seamless transitions and high-quality outcomes.

Key Responsibilities:
  • Identify and prioritize patients in need of care management services, utilizing a holistic approach that considers biopsychosocial, functional, cultural, spiritual, and financial factors.
  • Collaborate with patients, families, caregivers, and healthcare teams to develop and implement personalized care plans, promoting quality and cost-effective outcomes.
  • Maintain accurate and timely documentation of patient information, including physical and functional limitations, psychosocial characteristics, educational needs, and transitional requirements.
  • Initiate referrals to relevant disciplines as needed, ensuring smooth transitions of care and optimal patient outcomes.
Requirements:
  • CURRENT RN LICENSE IN THE STATE IN WHICH THEY ARE WORKING OR COVERED BY COMPACT
  • PREFERRED CASE MANAGEMENT CERTIFICATION (ACM, CCM, OR ANCC) UPON HIRE
  • ASSOCIATE DEGREE IN NURSING REQUIRED, WITH BSN STARTED WITHIN 2 YEARS AND COMPLETED WITHIN 5 YEARS
  • 1 YEAR OF EXPERIENCE IN A CLINICAL SETTING REQUIRED, WITH 1-2 YEARS OF ACUTE CARE EXPERIENCE PREFERRED
