We are seeking a skilled Discharge Planner RN to join our team, responsible for coordinating the care of assigned patients through collaboration with physicians, nursing staff, and other healthcare professionals.

Key Responsibilities:

  • Evaluate the appropriate utilization of services and facilitate optimal physician documentation and assignment of optimal DRG and coding assignment to reflect case mix index.
  • Conduct concurrent and retrospective clinical review of medical records to ensure efficient utilization of clinical resources and optimal clinical and continuing care.
  • Collaborate with healthcare professionals to facilitate smooth transitions of care and optimize patient outcomes.

Requirements:

  • Graduate from a School of Nursing with a current license from the Alabama Board of Nursing.
  • BSN preferred, Case Manager certification strongly preferred.
  • At least two (2) years of hospital patient care experience required.
  • Previous experience in home care, reimbursement, or hospital insurance preferred.

Preferred Qualifications:

  • Previous inpatient hospital case management experience, including utilization review, home care, and social services.
  • Strong oral, written, and interpersonal skills.
  • Ability to work independently with minimal supervision.
