As a Discharge Planner, you will play a critical role in coordinating care for patients, working closely with physicians, nursing staff, and other healthcare professionals to ensure seamless transitions and optimal outcomes.

Key Responsibilities:
  • Assess patient needs and develop personalized care plans to facilitate efficient utilization of clinical resources.
  • Collaborate with interdisciplinary teams to coordinate care, including skilled nursing facilities, home health, hospice, and rehabilitation services.
  • Ensure timely and effective communication with patients, families, and healthcare providers to promote continuity of care.
  • Monitor patient progress, identifying areas for improvement and implementing strategies to enhance care coordination.
  • Maintain accurate and up-to-date records of patient interactions, care plans, and outcomes.

Bachelor's degree in a related field and three years of hospital experience preferred. Knowledge of case management and certification in case management (CCM or ACM) a plus. Strong communication, interpersonal, and organizational skills required. Ability to work independently with minimal supervision and prioritize workload to achieve maximum efficiency.
