Main Responsibilities:**

  • Conduct comprehensive assessments of patients' social, emotional, and environmental factors to identify potential barriers to care.
  • Develop and implement discharge plans in collaboration with the interdisciplinary treatment team, focusing on maintaining or improving social supports and patient safety.
  • Provide clinical supervision to peers and students, promoting best practices and optimizing patient outcomes.
  • Evaluate the effectiveness of interventions and adjust strategies as needed to ensure optimal patient outcomes.
  • Collaborate with the healthcare team to enhance quality of care and efficiency, facilitating seamless transitions for patients throughout the continuum of care.

Requirements:**

  • Bachelor's degree in Social Work (required); Master's degree in Social Work or a related field (preferred).
  • 1 year of experience in a clinical setting, with expertise in casework principles, diagnosis, assessment, crisis intervention, and treatment planning.
  • Knowledge of community resources, public assistance systems, Medicare, and Medicaid.
  • Certification or licensure in the field of study (as required by state and federal law).
