Position Overview: NewCourtland is seeking a highly skilled and compassionate professional to support our housing with supportive services program. The ideal candidate will have extensive experience working with adults dealing with substance abuse, homelessness, and behavioral health issues.

Key Responsibilities:

  • Conduct comprehensive assessments to determine clients' needs and develop individualized care plans.
  • Provide counseling and support services to clients dealing with substance abuse, homelessness, and behavioral health challenges.
  • Collaborate with a multidisciplinary team to coordinate and deliver comprehensive care.
  • Facilitate access to community resources, including housing, medical care, employment services, educational opportunities, and financial management.
  • Develop and implement evidence-based interventions to support clients' recovery and stability.
  • Maintain detailed and accurate records of client progress and services provided.
  • Advocate for clients' rights and needs within the community and with service providers.
  • Participate in regular training and professional development to stay current with best practices in social work and supportive services.

Qualifications:

  • Bachelor's degree in social work from an accredited institution, preferred.
  • Minimum of 3 years of experience working with adults in substance abuse, homelessness, and behavioral health settings.
  • Excellent assessment, counseling, and case management skills.
  • Ability to work effectively in a team-oriented, collaborative environment.
  • Exceptional communication and interpersonal skills.
