Job Summary

We're seeking a dedicated Hospice Registered Nurse (RN) Case Manager to join our team for a full-time position. As a Hospice RN Case Manager, you'll play a crucial role in planning, organizing, and directing hospice care.

Key Responsibilities
  • Conduct patient and family assessments
  • Develop and implement personalized care plans
  • Provide skilled nursing care and pain management
  • Coordinate with interdisciplinary team members
  • Maintain accurate patient records
  • Participate in on-call duties
Qualifications
  • RN license
  • 1-2 years recent acute care experience (preferred)
  • 1-2 years home care experience (preferred)
  • Valid driver's license and reliable vehicle
  • Strong communication and interpersonal skills
  • Empathy and comfort with end-of-life care
Work Environment
  • Mix of facility and home-based care
  • Average caseload: 12-15 patients
  • Monday – Friday, 8am-5pm (some on-call and weekends)
  • Centralized territory assignment
