Job Summary

VNA Health Care seeks a skilled Family Nurse Practitioner to provide comprehensive medical evaluation, education, and management for clients across the lifespan. The ideal candidate will work collaboratively with a VNA Health Center physician to deliver high-quality, patient-centered care in a community-based setting.

Key Responsibilities:

  • Assess and interpret medical and psycho-social histories, perform physical examinations, and identify problems.
  • Formulate a plan of treatment, provide education, and refer clients as indicated.
  • Consult with or refer clients with abnormal conditions or other medical, psychological, or social problems to the collaborating physician or other appropriate referral source.
  • Order, perform, and interpret laboratory tests as part of the physical examination and health management.
  • Report communicable disease in a timely manner to the appropriate Health Department.
  • Conduct health teaching and counseling during the physical exam, enabling clients to make informed choices about health and treatment options.
  • Complete client clinical record documentation in a timely manner, according to applicable policies and procedures.
  • Apply exposure control procedures and understand medical emergency procedures.
  • Maintain professionalism and adhere to client confidentiality in all VNA Health Center practices.
  • Participate in quality improvement activities, including chart audits, outcome studies, client satisfaction surveys, and proficiency testing for CLIA.
  • Familiarize yourself with VNA Health Center policies, procedures, and protocols, and assist in the review and development of standing orders, policies, procedures, and protocols.
  • Provide consultative direction as needed for nursing, laboratory, and clinic assistant staff.
  • Identify and participate in continuing education programs to enhance professionalism and ensure quality of care.
  • Participate in quality improvement/assurance activities and provide on-call phone coverage as scheduled.
  • Order medications covered by scope of practice.
  • Familiarize yourself with VNA Health Center grant programs and regulatory guidelines.
