WelbeHealth is seeking a compassionate and dedicated Home Care Assistant - Patient Care Specialist to join our team in Menlo Park, CA. As a Home Care Assistant, you will play a vital role in providing high-quality patient care and support in the comfort of our patients' homes.

Key Responsibilities:
  • Provide hands-on personal care and assistance with activities of daily living (ADLs) to patients, including bathing, dressing, grooming, and toileting.
  • Assist patients with Instrumental Activities of Daily Living (IADLs), such as laundry, light housekeeping, and shopping.
  • Aid patients with safe transfer in and out of their homes and act as a medical escort on vehicles and to medical appointments as assigned.
  • Help patients with light meal preparation and feeding, ensuring they are nourished and hydrated.
Requirements:
  • High School Diploma or Equivalency
  • Reliable means of transportation
  • Bilingual preferred
  • Must be registered and cleared with the Department of Social Services Home Care Registry prior to start date
