Requirements
  • Active Pharmacy License: Must hold an active pharmacy license in the state where employed.
  • Not on DEA Excluded Parties List: Must not be listed on the DEA Excluded Parties List.
  • Immunization Certification: Must hold immunization certification through an accredited organization.
  • Pharmacist in Charge: Must be listed as the pharmacist in charge on the pharmacy state license.
  • PMP Registration: Must submit required information/documents to the state PMP administrator to register for PMP access.
  • Education: Must hold a Bachelor of Science in Pharmacy or Pharm. D. degree.
