Position Overview

The Licensed Speech Language Pathologist will deliver speech therapy services to patients as directed by their healthcare provider. This role also includes offering consultation to Agency personnel and the patient's physician regarding speech therapy as necessary.

The Speech Language Pathologist will engage in the assessment, planning, and execution of rehabilitation services for individuals with speech, language, and/or swallowing challenges. This includes conducting evaluations of speech and language capabilities through diagnostic assessments.

Responsibilities also encompass documenting and communicating the client's treatment progress and any alterations in the patient's condition to the attending physician, as well as educating family members and healthcare team members on strategies to enhance the patient's communication skills. Maintaining comprehensive clinical and progress records is essential.


Core Responsibilities

  • Conduct assessments, develop plans, and implement rehabilitation services for speech, language, and/or swallowing impairments.
  • Perform evaluations of speech and language skills through diagnostic assessments, setting goals and predicting recovery outcomes.
  • Document and report the client's treatment progress and any changes in the patient's condition to the physician.
  • Educate family members and healthcare team members on methods to assist the patient in enhancing communication abilities.
  • Reassess the patient's progress and the established rehabilitation plan as frequently as required.
  • Prepare and submit periodic reports, as mandated by the Agency, to the physician and third-party payers regarding the care provided.
  • Facilitate appropriate referrals to external audiology speech clinics or other community resources as necessary.
  • Maintain thorough clinical and progress documentation and ensure timely submission of all required paperwork in accordance with Agency policies.
  • Act as a liaison and follow up with other community organizations.
  • Participate in in-service training, case discussions, and staff meetings as required.
  • Continuously enhance professional skills through in-service training and similar professional development opportunities.
  • Perform any additional duties as assigned by the supervisor.

Work Schedule and Environment

  • The employee will be required to travel daily for meetings and/or visits to various residential settings based on their assigned caseload.

Travel Requirements

  • The employee will travel by car and may encounter varying weather conditions.
