Social Services Coordinator

Job Summary:
We are seeking a skilled Social Services Coordinator to join our team at Claxton-Hepburn Medical Center. The successful candidate will be responsible for evaluating patient needs, developing discharge plans, and coordinating care with healthcare providers and community resources.
Key Responsibilities:
- Evaluate patient needs and develop discharge plans in collaboration with healthcare providers and family members
- Coordinate care with community resources, including home health agencies, long-term care facilities, and hospice services
- Facilitate family healthcare decision-making and provide emotional support to patients and families
- Consult with ED Case Managers and Providers on complex discharge planning cases
- Initiate referrals to community resources and long-term care facilities
Requirements:
- Associates or higher degree in healthcare or human services
- One year prior relevant work experience in discharge planning or care coordination
- Proficiency with computer software programs
- Great organizational and time management skills
- Effective verbal and written communication skills
Work Environment:
This is a per diem position working day shifts from 7:30am to 4pm. The successful candidate will be required to bend, lift, and stand for extended periods.
