Job Summary

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at Phoenix Senior Living. As an LPN, you will play a vital role in providing high-quality care and support to our residents, ensuring their physical, emotional, and social well-being.

Key Responsibilities
  • Resident Care
    • Conduct thorough assessments of residents' needs and develop personalized care plans to meet their unique requirements.
    • Administer medications and treatments as prescribed by physicians, ensuring accurate documentation and adherence to regulatory guidelines.
    • Collaborate with the Wellness Director to ensure seamless care transitions and effective communication with residents, families, and healthcare teams.
    • Monitor and report any changes in residents' conditions, including medication needs, to the Wellness Director and Executive Director.
  • Leadership and Support
    • Provide leadership and guidance to nursing staff, promoting a culture of excellence and teamwork.
    • Assist the Wellness Director in maintaining accurate and up-to-date resident records, including medication profiles and treatment plans.
    • Support the development and implementation of quality improvement initiatives, ensuring compliance with regulatory requirements and Phoenix Senior Living's standards.
  • Communication and Collaboration
    • Communicate effectively with residents, families, and healthcare teams to ensure seamless care coordination and address any concerns or issues.
    • Collaborate with the pharmacy to ensure accurate medication management and timely refills.
Requirements
  • Education and Experience
    • Current LPN licensure in the state of practice.
    • A minimum of one year of experience working in a skilled nursing or assisted living setting, preferably with elders or individuals with disabilities.
  • Skills and Abilities
    • Strong leadership and communication skills, with the ability to work effectively in a fast-paced environment.
    • Proficiency in using electronic medical records and other healthcare software.
    • Knowledge of infection control procedures and regulatory requirements.
    • Ability to work flexible hours, including weekends and evenings, to meet the needs of our residents.
Physical Requirements

The LPN will be required to stand, walk, and sit for extended periods, with occasional lifting and carrying of up to 50 pounds. The ability to drive and use hands for repetitive tasks, such as simple grasping and pushing, is also required.
