Job Summary

St. David's Healthcare at Home is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team. As a Home Health LPN, you will provide high-quality patient care in the comfort of their own homes, working collaboratively with our interdisciplinary team to develop and implement individualized care plans.

Responsibilities
  • Administer skilled nursing care to patients in their homes, following physician orders and the Plan of Care.
  • Conduct ongoing assessments to identify patient needs and develop effective care plans.
  • Communicate with patients, families, and healthcare team members to ensure seamless care coordination.
  • Provide education and support to patients and caregivers on disease management, medication administration, and other health-related topics.
  • Collaborate with the healthcare team to identify and address any barriers to care.
Requirements
  • Graduate of an accredited LPN/LVN program.
  • Minimum of one year experience as an LPN/LVN in a clinical care setting.
  • Home care experience preferred.
  • Must be licensed as an LPN/LVN in good standing in the state(s) where they will be providing services.
  • Current BCLS certification.
  • Reliable transportation and valid driver's license.
