Job Summary:The RN Case Manager 1 provides discharge planning and continuity of care for assigned patients in acute and post-acute settings. This includes coordinating services, acting as a liaison between patients, families, and healthcare team members, and utilizing management techniques to determine the medical necessity and appropriateness of healthcare services.
Responsibilities:- Collects and analyzes data to identify at-risk populations and develops strategies to improve patient outcomes.
- Collaborates with interdisciplinary healthcare teams to ensure effective patient management and safe care.
- Provides educational resources and referrals to patients and families to address identified needs.
- Acts as an advocate for patients to resolve barriers to care progression.
- Participates in clinical performance improvement activities to drive practice changes and positively impact outcomes.
- Bachelor of Science in Nursing degree.
- Minimum of one year Case Management experience and/or Clinical Care experience.
- Basic Life Support through the American Heart Association.
- Licensed in the Commonwealth of Virginia to practice as a Registered Nurse (RN).
