Family Centered Clinician Role

As a Family Centered Clinician at Sevita, you will provide home-based, empirically based therapy to youth and their families. This role requires a strong understanding of family dynamics and the ability to develop and implement effective treatment plans.

Key Responsibilities:
  • Complete diagnostic evaluations to assess the needs of youth and their families.
  • Develop treatment plans based on assessment of strengths, needs, and collaboration with referral agencies.
  • Provide activity and participation-based interventions and approaches developed from training materials, experience, and individual and/or families' successes.
  • Make referrals to services and resources on behalf of individuals and/or families served.
  • Ensure that documentation and records are kept confidential, organized, and submitted to supervisor in an accurate and timely manner.
  • Participate in team meetings with peers and collateral team meetings when needed.
  • Support peer development by addressing areas of concern for professional development and team performance issues.
  • Provide an aftercare plan for each family/individual served.
  • Build and maintain positive relationships with community partners from referring/collateral agencies.
Qualifications:
  • LPC, MFT, or LSW required.
  • Two years related field experience.
  • Specialized training required and provided.
  • Exceptional communication skills with an ability to establish trust and rapport quickly.
  • A good listener with an ability to empathize while still providing guidance.
