We are seeking a compassionate and skilled Licensed Vocational Nurse to join our team at St. Gabriel's Hospice and Palliative Care in the San Angelo area.

About the Role:

As a Licensed Vocational Nurse, you will provide high-quality patient care under the supervision of a registered nurse. Your primary function will be to coordinate care with the Interdisciplinary Team, patient, family, and referring agency.

Responsibilities:
  • Provide nursing care to patients of all ages in their place of residence
  • Coordinate care with the Interdisciplinary Team, patient, family, and referring agency
  • Assist in identifying patient needs and developing care plans
  • Apply concepts of infection control and standard precautions in patient care activities
  • Ensure continuity of care through delivery of quality patient care
  • Provide effective communication to patients, families, and healthcare professionals
Requirements:
  • Graduate of an accredited School of Vocational/Practical Nursing
  • State license as a Licensed Vocational/Practical nurse, or a multi-state license issued by a member of the National Nurse Licensure Compact (NLC)
  • Current driver's license
  • Two years of experience as a Licensed Vocational/Practical Nurse in a clinical care setting, with hospice experience preferred
