Senior Nurse Navigator

City of Hope is seeking a compassionate and skilled Senior Nurse Navigator to join our team in Newport Beach. As a key member of our Community Practice, you will play a vital role in ensuring that patients receive timely and high-quality care throughout their cancer journey.

Key Responsibilities:
  • Facilitate the entry of new patients into the healthcare system, triaging them to the appropriate services
  • Assist with determining and assessing required medical records, imaging, and pathology to expedite the new patient visit and review records upon receipt
  • Provide clinical trial overviews, specific information as requested, and screening when needed
  • Ensure all new patient education is completed prior to the first visit
  • Assist new patients with completing all necessary documents and participate in nursing new patient intake documentation completion
  • Assess and address patient care barriers
  • Provide ongoing education, resources, and referrals both internal and external
  • Serve as the primary contact for patient issues associated with the coordination of their care
  • Routinely interact with the care team to address patients' questions and care coordination concerns
  • Facilitate shared decision-making, ensuring patients are fully informed of risks and benefits of treatment options and integrating their values and practices regarding treatment decisions
  • Promote advance care planning with patients as appropriate
  • Track and monitor diagnosis and treatment information, outcomes, and patient contacts for measuring and improving quality of care for the individual patient and patient population
  • Support the use of palliative care and other downstream services by assessing for late and long-term side effects and other physical barriers to the patient's quality of life
  • Provide service to our stakeholders, including patients, caregivers, colleagues, and each other in a safe, courteous, accountable, efficient, and innovative manner
  • Meet with patients via telehealth and/or in person as needed
  • Maintain current working knowledge and understanding of the oncology field, enhancing professional growth and development through participation in educational programs, professional affiliations, current literature, in-service meetings, and workshops
  • Be proficient in and utilize the electronic medical record for documentation of care
  • Consistently document all assigned patient touch points
  • Provide reports and organize data as requested
Qualifications:
  • Bachelor's degree
  • Five years of professional nursing experience, including three years of ambulatory oncology experience
  • Registered Nurse in the State of California
  • One of the following certifications required within one year of hire: OCN, AOCN, ONN-CG, ONN-CGSM, AOCNP
